Endorsement Proposals

FOGS wishes to provide services to member offices, which benefit the member directly. However, we will be very selective as to which services we will market to our members. Please complete and submit this form according to the type of proposal you wish to submit to FOGS. After selecting the appropriate option, you will be asked to agree to specific criteria. Once all criteria have been met, the administrative office or FOGS Board, as appropriate, for consideration and approval will review the proposal.

Once all criteria have been met, the FOGS board or administrative staff will consider the application and you will be notified of their decision.

While routine requests generally can be processed in 72 hours, other items requiring board approval may take up to three months for a response.
